Connected Care Case Management
What is Case Management?
Case Management is your opportunity to partner with a health professional who assists you in coordinating your care.Once enrolled the service may include:
• Creation of a personalized plan based upon your own identified concerns
• Coordination of care between your primary doctor, pharmacy, and specialists who are involved in your care
• Health coaching and education regarding your chronic conditions
• Medication review and assistance
• Help connecting you to available resources in your community
How to get started
• Ask your doctor about the program or your doctor might refer you to the program• A Case Manager will call you and explain the service in more detail
• When you enroll in the program your Case Manager will work with you to create a patient-centered care plan, act as your health coach, and assist with all your care coordination needs

• Arthritis• Alzheimer's
• High Blood Pressure
• Heart Disease
• COPD
• Pulmonary Fibrosis
• Parkinson's Disease
• Congestive Heart Failure
• Obesity
• Depression
• Bipolar Disorder
• Cancer diagnosis
• Diabetes
• Atrial Fibrillation
Services Include
• Personalized assistance from dedicated health care professionals to create your care plan with you• Case Manager phone calls with you between clinic visits to answer your questions and help you with managing your care
• Connect you with other resources and services that can help you stay healthy
• Coordination of care, including helping with referrals and communication between your primary care provider and other providers
• Medication and self-care support
• Personalized Health Coaching
to help you reach your health care goals
Additional Information:
• Syringa is now offering care management regardless of insurance type. If you have Medicare, VA/Tricare, or Blue Cross a small monthly copay will apply• Participation is voluntary and you may withdraw from this program at any time
